1) Your Name

2) Medical Condition(s)

List your most important medical condition — the one responders need to know right away. If you have more than one, choose the two or three that are most critical. Examples include:

  • Epilepsy / Seizure Disorder
  • Heart Disease / Pacemaker
  • Asthma or COPD
